Getting started is easy, just fill in the information below and we will do the rest!
If you aren’t transferring any prescriptions and you want to get started you can still use this form, or if you prefer give us a call or send us a text at 519-954-1055.
If you send us a text with a picture of your prescription we can prepare it as well. We can then collect the original copy upon delivery or when you pick up your prescription.
